Office Policies
Registration
On your first visit, a permanent record will be initiated. Your cooperation in providing the requested data on the patient registration form is appreciated. All your records will be treated confidentially. If another physician has referred you to this office (family doctor, internist, pediatrician, HMO primary care physician, etc.) be sure to include this information on the registration form, as we will forward a consultation report.
Referrals
It is the responsibility of patients to obtain referrals from HMO primary care physicians prior to visits to our office. If a referral has not been authorized, you will be asked to sign a waiver accepting responsibility for payment.
Appointments
Patients are seen by appointment, although accommodations will be made for emergency problems. Appointments are scheduled Monday through Friday in the Concord office from 7:30 a.m. to 4:30 p.m. and in the Cambridge office from 8:00 a.m. to 4:30 p.m.
Phototherapy treatments are scheduled in the Concord office on Monday, Tuesday Wednesday and Friday from 6:30 a.m. to noon, and on Monday, Wednesday and Friday from 1 p.m. to 5:45 p.m., and in the Cambridge office on Monday, Wednesday and Friday from 7 a.m. to 3:45 p.m. If you are unable to keep a scheduled appointment, please inform the receptionist or phototherapist in advance.
A physician is available at night and on weekends to care for dermatologic emergencies. We ask, however, that routine problems, such as medication refills, be handled during the weekday office hours so that we can refer to patient records for appropriate recommendations.
We will certainly try to see you at your appointment time. Please keep in mind that emergencies, complicated patient problems, involved surgical procedures, and patient or referring physician telephone calls may result in unforeseeable delays. The same additional time courtesy will be extended to you should your problem require more than the allotted time.
